Acellular Dermal Matrices

Breast implants need support from your natural tissues in order to stay in the correct position on your chest. Sometimes, your own tissues are not strong enough to support the size of implant chosen for your augmentation or reconstruction. Over time, your tissues may weaken or stretch, resulting in deformities and asymmetries. These gradual changes in your breasts can affect your overall look. Dr. Strawn will often use acellular dermal matrices for breast revision or breast reconstruction to reinforce and strengthen the breast tissue for long-term support.

What Are Acellular Dermal Matrices?

An acellular dermal matrix (ADM) is a soft tissue substitute comprised of real tissues. ADMs retain the collagen, our body’s primary support protein, which then becomes integrated into your own tissues. ADMs create a strong layer along the breast’s borders that helps maintain the strength and integrity of your entire breast over time.

ADMs for Implant Malposition

The weakening of breast tissue is the most common cause of many implant malposition issues. As the breast tissue holding the implant begins to lose its durability, your implants can slowly shift their position medially, laterally, or vertically. Typical forms of implant malposition include:
  • Bottoming out – the implant slips below the inframammary fold
  • Symmastia – one or both implants shift toward the middle of the chest
  • Lateral shifting – one or both implants move into the armpit area
  • Asymmetry – one implant sits lower than the other

ADMs for Breast Reconstruction

ADMs can be used during breast reconstruction to provide additional tissue support in patients with thinner or weaker breast tissue or if tissue was damaged during the mastectomy. Dr. Strawn prefers to use your own serratus anterior muscle fascia as a first line of support for the tissue expanders. However, there are times when utilizing an ADM will assist in achieving needed support. Ultimately this may result in a more natural, sustainable breast appearance. Since an ADM integrates with your own breast tissue, a secondary surgery is not required.
